Sports Physicals

Sports physicals for the 2021-2022 school year may not be authorized earlier than June 1 and shall be effective, regardless of when performed during a school year, until the latter of the next May 31 or the conclusion of the spring sports season. Students MUST submit their PIAA physical forms before they participate in any sports programs. On the day the physical is completed, ask the physician to stamp and date Section 6 of the PIAA physical form. We will only accept the completed PIAA physical form for participation in afterschool athletic programs.
